Project Overview
A regional healthcare provider was running their telehealth operations on a legacy PHP monolith built in 2015. The system had no API layer, brittle database queries, zero automated testing, and frequent downtime incidents.
After failing a HIPAA compliance audit, they needed a modern platform — fast. Syslice was brought in to architect and deliver a complete rebuild without disrupting ongoing patient care.
The Challenge
The existing system could not handle the surge in telehealth demand. Appointment scheduling took 8+ minutes, video calls dropped regularly, and patient data handling had compliance gaps.
The migration had to happen while the old system remained operational. Zero downtime during the transition was non-negotiable — patients relied on this platform daily.
Our Solution
We designed a phased migration strategy with four workstreams running in parallel.
Microservices Architecture
Decomposed the monolith into 12 independently deployable services with clear domain boundaries and API contracts.
HIPAA-Compliant Infrastructure
AWS HIPAA-eligible services with encryption at rest and in transit, BAA coverage, access logging, and automated audit trails.
Smart Scheduling Engine
AI-assisted scheduling that considers provider availability, patient preferences, and appointment type to reduce booking time by 60%.
Parallel Migration
Ran old and new systems in parallel with data sync, allowing gradual patient migration without any service interruption.
